- Comment Eine disseminierte intravaskuläre Gerinnung (DIC [disseminated intravascular coagulation]) – im deutschen Sprachgebrauch oft auch synonym Verbrauchskoagulopathie genannt (Lasch 1971) -, ist ein erworbenes komplexes Syndrom auf dem Boden unterschiedlichster Erkrankungen. Sie ist daher immer ein sekundäres Geschehen. In ihrem milden, kompensierten Verlaufsformen ist sie oft asymptomatisch, in ihren schweren Formen hat sie jedoch eine hohe Mortalität. Kennzeichen der DIC ist die abnorme intravasale Thrombinbildung, die vorwiegend in der Mikrozirkulation eine diffuse Fibrinbildung bedingt. Dadurch kommt es zu einem erhöhten Umsatz („Verbrauch“) an Thrombozyten, Fibrinogen, Gerinnungsfaktoren und Gerinnungsinhibitoren sowie einem Anstieg von Aktivierungsmarkern der Gerinnung und Fibrinolyse.
